Annual Impact Report

FRGP is very interested in learning about the activities being supported by its grant funding. Accordingly, it typically conducts one site visit during each year of a grant term and asks each grant recipient to complete an annual impact report.

Impact reports can be found and submitted in the online grants portal, and an email reminder will be sent to the grant contact two weeks prior to the deadline. (To decrease the likelihood of missing such emails, applicants are encouraged to check their spam folders regularly for emails from administrator@grantinterface.com.) The report is required for an organization to be eligible for future installments of current grants as well as future new grants.

For planning purposes, the impact report questions are listed below. Please note, however, that neither hard copies nor email submissions of the impact report can be accepted as compliant.

The chart below outlines the impact report and grant installment schedule for each cycle. For a brief period, Felicia Rose Grant Program operated on a quarterly basis. Starting in 2025, it will move to twice a year in Quarters 2 and 4.

For example, if your organization applied during the 2024 Quarter 2 Cycle, the grant would be awarded on June 1, 2024 and disbursed on June 15, 2024. The first impact report would be due on May 1, 2025 and the subsequent disbursement made on June 15, 2025. The final impact report would be due on May 1, 2026.

Please note: all impact reports must be submitted in order for your organization to be considered for future funding.

Nonprofits are strongly encouraged to plan ahead and submit well before the deadline, as extensions will not be granted. If declined, a nonprofit may submit an additional application during the same grant cycle the following year, or later.

Progress and Results*

Please briefly describe (in 300 words or fewer) the key actions, activities, or products resulting from the grant funding, as well as any resulting benefit to the organization or the people it serves.

Grant Budget*

Please detail how your organization expended its most recent grant installment. If you do not anticipate spending all the funds within one year of the grant payment, please include the expected "rollover" amount, as well as your future plans for those funds.
If you requested funds for general operating support, please simply indicate that below. It is not necessary to detail expenditures in such instances.

Variance*

Please explain any significant variance from your planned use of funds. For example, have there been changes in your project timeline or expenditures? If a Grant Amendment Request form was previously submitted and approved, please note that below. It is not necessary, however, to reiterate the details here.

Challenges*

Please describe the challenges, if any, the funded project has encountered, and explain how you overcame them (or plan to overcome them). For example, have other anticipated funding sources for this project not materialized?

Organizational Changes*

What significant changes, if any, have occurred at your organization since the beginning of the grant term? For example, has there been a change in leadership or staff responsible for this project, or is a merger being contemplated?

Annual Revenue*

Please enter the organization’s total revenue for the three most recently completed fiscal years. NOTE: In-kind income should NOT be included. (The numbers should represent the entire organization, not just the department, office, or program related to this application.) To aid the report reviewers, please format your response like the example below:
